3D Brows and Wellness is proud to work with Prevea360 Health Plan as a non-plan provider for 3D Areola Restoration Services. If you are a Prevea360 Health Plan member, you would need to obtain prior authorization for services from your practitioner. From there, a free consultation with 3D Brows and Wellness would be required before scheduling an appointment for service.
Don't See Your Insurance?
If your carrier is not listed, you are welcome to self-pay. You may be able to receive reimbursement for the services from your insurance company. In this case I am happy to provide you with receipts to file the claim.
